The very word for many of you conjures unhappy memories of boredom
loading
in high school biology or physics class.
loading
But let me assure that what you did there
loading
had very little to do with science.
loading
That was really the "what" of science.
loading
It was the history of what other people had discovered.
loading
What I'm most interested in as a scientist
loading
is the "how" of science.
loading
Because science is knowledge in process.
loading
We make an observation, guess an explanation for that observation,
loading
and then make a prediction that we can test
loading
with an experiment or other observation.
loading
A couple of examples.
loading
First of all, people noticed that the Earth was below, the sky above,
loading
and both the Sun and the Moon seemed to go around them.
loading
Their guessed explanation
loading
was that the Earth must be the center of the universe.
loading
The prediction: everything should circle around the Earth.
loading
This was first really tested
loading
when Galileo got his hands on one of the first telescopes,
loading
and as he gazed into the night sky,
loading
what he found there was a planet, Jupiter,
loading
with four moons circling around it.
loading
He then used those moons to follow the path of Jupiter
loading
and found that Jupiter also was not going around the Earth
loading
but around the Sun.
loading
So the prediction test failed.
loading
And this led to the discarding of the theory
loading
that the Earth was the center of the universe.
loading
Another example: Sir Isaac Newton noticed that things fall to the Earth.
loading
The guessed explanation was gravity,
loading
the prediction that everything should fall to the Earth.
loading
But of course, not everything does fall to the Earth.
loading
So did we discard gravity?
loading
No. We revised the theory and said, gravity pulls things to the Earth
loading
unless there is an equal and opposite force in the other direction.
loading
This led us to learn something new.
loading
We began to pay more attention to the bird and the bird's wings,
loading
and just think of all the discoveries
loading
that have flown from that line of thinking.
loading
So the test failures, the exceptions, the outliers
loading
teach us what we don't know and lead us to something new.
loading
This is how science moves forward. This is how science learns.
loading
Sometimes in the media, and even more rarely,
loading
but sometimes even scientists will say
loading
that something or other has been scientifically proven.
loading
But I hope that you understand that science never proves anything
loading
definitively forever.
loading
Hopefully science remains curious enough
loading
to look for
loading
and humble enough to recognize
loading
when we have found
loading
the next outlier,
loading
the next exception,
loading
which, like Jupiter's moons,
loading
teaches us what we don't actually know.
loading
We're going to change gears here for a second.
loading
The caduceus, or the symbol of medicine,
loading
means a lot of different things to different people,
loading
but most of our public discourse on medicine
loading
really turns it into an engineering problem.
loading
We have the hallways of Congress,
loading
and the boardrooms of insurance companies that try to figure out how to pay for it.
loading
The ethicists and epidemiologists
loading
try to figure out how best to distribute medicine,
loading
and the hospitals and physicians are absolutely obsessed
loading
with their protocols and checklists,
loading
trying to figure out how best to safely apply medicine.
loading
These are all good things.
loading
However, they also all assume
loading
at some level
loading
that the textbook of medicine is closed.
loading
We start to measure the quality of our health care
loading
by how quickly we can access it.
loading
It doesn't surprise me that in this climate,
loading
many of our institutions for the provision of health care
loading
start to look a heck of a lot like Jiffy Lube.
loading
(Laughter)
loading
The only problem is that when I graduated from medical school,
loading
I didn't get one of those little doohickeys
loading
that your mechanic has to plug into your car
loading
and find out exactly what's wrong with it,
loading
because the textbook of medicine
loading
is not closed.
loading
Medicine is science.
loading
Medicine is knowledge in process.
loading
We make an observation,
loading
we guess an explanation of that observation,
loading
and then we make a prediction that we can test.
loading
Now, the testing ground of most predictions in medicine
loading
is populations.
loading
And you may remember from those boring days in biology class
loading
that populations tend to distribute
loading
around a mean
loading
as a Gaussian or a normal curve.
loading
Therefore, in medicine,
loading
after we make a prediction from a guessed explanation,
loading
we test it in a population.
loading
That means that what we know in medicine,
loading
our knowledge and our know-how,
loading
comes from populations
loading
but extends only as far
loading
as the next outlier,
loading
the next exception,
loading
which, like Jupiter's moons,
loading
will teach us what we don't actually know.
loading
Now, I am a surgeon
loading
who looks after patients with sarcoma.
loading
Sarcoma is a very rare form of cancer.
loading
It's the cancer of flesh and bones.
loading
And I would tell you that every one of my patients is an outlier,
loading
is an exception.
loading
There is no surgery I have ever performed for a sarcoma patient
loading
that has ever been guided by a randomized controlled clinical trial,
loading
what we consider the best kind of population-based evidence in medicine.
loading
People talk about thinking outside the box,
loading
but we don't even have a box in sarcoma.
loading
What we do have as we take a bath in the uncertainty
loading
and unknowns and exceptions and outliers that surround us in sarcoma
loading
is easy access to what I think are those two most important values
loading
for any science:
loading
humility and curiosity.
loading
Because if I am humble and curious,
loading
when a patient asks me a question,
loading
and I don't know the answer,
loading
I'll ask a colleague
loading
who may have a similar albeit distinct patient with sarcoma.
loading
We'll even establish international collaborations.
loading
Those patients will start to talk to each other through chat rooms
loading
and support groups.
loading
It's through this kind of humbly curious communication
loading
that we begin to try and learn new things.
loading
As an example, this is a patient of mine
loading
who had a cancer near his knee.
loading
Because of humbly curious communication
loading
in international collaborations,
loading
we have learned that we can repurpose the ankle to serve as the knee
loading
when we have to remove the knee with the cancer.
loading
He can then wear a prosthetic and run and jump and play.
loading
This opportunity was available to him
loading
because of international collaborations.
loading
It was desirable to him
loading
because he had contacted other patients who had experienced it.
loading
And so exceptions and outliers in medicine
loading
teach us what we don't know, but also lead us to new thinking.
loading
Now, very importantly,
loading
all the new thinking that outliers and exceptions lead us to in medicine
loading
does not only apply to the outliers and exceptions.
loading
It is not that we only learn from sarcoma patients
loading
ways to manage sarcoma patients.
loading
Sometimes, the outliers
loading
and the exceptions
loading
teach us things that matter quite a lot to the general population.
loading
Like a tree standing outside a forest,
loading
the outliers and the exceptions draw our attention
loading
and lead us into a much greater sense of perhaps what a tree is.
loading
We often talk about losing the forests for the trees,
loading
but one also loses a tree
loading
within a forest.
loading
But the tree that stands out by itself
loading
makes those relationships that define a tree,
loading
the relationships between trunk and roots and branches,
loading
much more apparent.
loading
Even if that tree is crooked
loading
or even if that tree has very unusual relationships
loading
between trunk and roots and branches,
loading
it nonetheless draws our attention
loading
and allows us to make observations
loading
that we can then test in the general population.
loading
I told you that sarcomas are rare.
loading
They make up about one percent of all cancers.
loading
You also probably know that cancer is considered a genetic disease.
loading
By genetic disease we mean that cancer is caused by oncogenes
loading
that are turned on in cancer
loading
and tumor suppressor genes that are turned off to cause cancer.
loading
You might think that we learned about oncogenes
loading
and tumor suppressor genes from common cancers
loading
like breast cancer and prostate cancer
loading
and lung cancer,
loading
but you'd be wrong.
loading
We learned about oncogenes and tumor suppressor genes
loading
for the first time
loading
in that itty-bitty little one percent of cancers called sarcoma.
loading
In 1966, Peyton Rous got the Nobel Prize
loading
for realizing that chickens
loading
had a transmissible form of sarcoma.
loading
Thirty years later, Harold Varmus and Mike Bishop discovered
loading
what that transmissible element was.
loading
It was a virus
loading
carrying a gene,
loading
the src oncogene.
loading
Now, I will not tell you that src is the most important oncogene.
loading
I will not tell you
loading
that src is the most frequently turned on oncogene in all of cancer.
loading
But it was the first oncogene.
loading
The exception, the outlier
loading
drew our attention and led us to something
loading
that taught us very important things about the rest of biology.
loading
Now, TP53 is the most important tumor suppressor gene.
loading
It is the most frequently turned off tumor suppressor gene
loading
in almost every kind of cancer.
loading
But we didn't learn about it from common cancers.
loading
We learned about it when doctors Li and Fraumeni
loading
were looking at families,
loading
and they realized that these families
loading
had way too many sarcomas.
loading
I told you that sarcoma is rare.
loading
Remember that a one in a million diagnosis,
loading
if it happens twice in one family,
loading
is way too common in that family.
loading
The very fact that these are rare
loading
draws our attention
loading
and leads us to new kinds of thinking.
loading
Now, many of you may say,
loading
and may rightly say,
loading
that yeah, Kevin, that's great,
loading
but you're not talking about a bird's wing.
loading
You're not talking about moons floating around some planet Jupiter.
loading
This is a person.
loading
This outlier, this exception, may lead to the advancement of science,
loading
but this is a person.
loading
And all I can say
loading
is that I know that all too well.
loading
I have conversations with these patients with rare and deadly diseases.
loading
I write about these conversations.
loading
These conversations are terribly fraught.
loading
They're fraught with horrible phrases
loading
like "I have bad news" or "There's nothing more we can do."
loading
Sometimes these conversations turn on a single word:
loading
"terminal."
loading
Silence can also be rather uncomfortable.
loading
Where the blanks are in medicine
loading
can be just as important
loading
as the words that we use in these conversations.
loading
What are the unknowns?
loading
What are the experiments that are being done?
loading
Do this little exercise with me.
loading
Up there on the screen, you see this phrase, "no where."
loading
Notice where the blank is.
loading
If we move that blank one space over
loading
"no where"
loading
becomes "now here,"
loading
the exact opposite meaning,
loading
just by shifting the blank one space over.
loading
I'll never forget the night
loading
that I walked into one of my patients' rooms.
loading
I had been operating long that day
loading
but I still wanted to come and see him.
loading
He was a boy I had diagnosed with a bone cancer a few days before.
loading
He and his mother had been meeting with the chemotherapy doctors
loading
earlier that day,
loading
and he had been admitted to the hospital to begin chemotherapy.
loading
It was almost midnight when I got to his room.
loading
He was asleep, but I found his mother
loading
reading by flashlight
loading
next to his bed.
loading
She came out in the hall to chat with me for a few minutes.
loading
It turned out that what she had been reading
loading
was the protocol that the chemotherapy doctors
loading
had given her that day.
loading
She had memorized it.
loading
She said, "Dr. Jones, you told me
loading
that we don't always win
loading
with this type of cancer,
loading
but I've been studying this protocol, and I think I can do it.
loading
I think I can comply with these very difficult treatments.
loading
I'm going to quit my job. I'm going to move in with my parents.
loading
I'm going to keep my baby safe."
loading
I didn't tell her.
loading
I didn't stop to correct her thinking.
loading
She was trusting in a protocol
loading
that even if complied with,
loading
wouldn't necessarily save her son.
loading
I didn't tell her.
loading
I didn't fill in that blank.
loading
But a year and a half later
loading
her boy nonetheless died of his cancer.
loading
Should I have told her?
loading
Now, many of you may say, "So what?
loading
I don't have sarcoma.
loading
No one in my family has sarcoma.
loading
And this is all fine and well,
loading
but it probably doesn't matter in my life."
loading
And you're probably right.
loading
Sarcoma may not matter a whole lot in your life.
loading
But where the blanks are in medicine
loading
does matter in your life.
loading
I didn't tell you one dirty little secret.
loading
I told you that in medicine, we test predictions in populations,
loading
but I didn't tell you,
loading
and so often medicine never tells you
loading
that every time an individual
loading
encounters medicine,
loading
even if that individual is firmly embedded in the general population,
loading
neither the individual nor the physician knows
loading
where in that population the individual will land.
loading
Therefore, every encounter with medicine
loading
is an experiment.
loading
You will be a subject
loading
in an experiment.
loading
And the outcome will be either a better or a worse result for you.
loading
As long as medicine works well,
loading
we're fine with fast service,
loading
bravado, brimmingly confident conversations.
loading
But when things don't work well,
loading
sometimes we want something different.
loading
A colleague of mine removed a tumor from a patient's limb.
loading
He was concerned about this tumor.
loading
In our physician conferences, he talked about his concern
loading
that this was a type of tumor
loading
that had a high risk for coming back in the same limb.
loading
But his conversations with the patient
loading
were exactly what a patient might want:
loading
brimming with confidence.
loading
He said, "I got it all and you're good to go."
loading
She and her husband were thrilled.
loading
They went out, celebrated, fancy dinner, opened a bottle of champagne.
loading
The only problem was a few weeks later,
loading
she started to notice another nodule in the same area.
loading
It turned out he hadn't gotten it all, and she wasn't good to go.
loading
But what happened at this juncture absolutely fascinates me.
loading
My colleague came to me and said,
loading
"Kevin, would you mind looking after this patient for me?"
loading
I said, "Why, you know the right thing to do as well as I do.
loading
You haven't done anything wrong."
loading
He said, "Please, just look after this patient for me."
loading
He was embarrassed --
loading
not by what he had done,
loading
but by the conversation that he had had,
loading
by the overconfidence.
loading
So I performed a much more invasive surgery
loading
and had a very different conversation with the patient afterwards.
loading
I said, "Most likely I've gotten it all
loading
and you're most likely good to go,
loading
but this is the experiment that we're doing.
loading
This is what you're going to watch for.
loading
This is what I'm going to watch for.
loading
And we're going to work together to find out if this surgery will work
loading
to get rid of your cancer."
loading
I can guarantee you, she and her husband
loading
did not crack another bottle of champagne after talking to me.
loading
But she was now a scientist,
loading
not only a subject in her experiment.
loading
And so I encourage you
loading
to seek humility and curiosity
loading
in your physicians.
loading
Almost 20 billion times each year,
loading
a person walks into a doctor's office,
loading
and that person becomes a patient.
loading
You or someone you love will be that patient sometime very soon.
loading
How will you talk to your doctors?
loading
What will you tell them?
loading
What will they tell you?
loading
They cannot tell you
loading
what they do not know,
loading
but they can tell you when they don't know
loading
if only you'll ask.
loading
